Summary Care Records

The NHS in England has over the past couple of years been introducing Summary Care Records to improve the safety and quality of patient care.

It will give healthcare staff faster, easier access to reliable information about you to help with your treatment.

Today, all the places where you receive care keep records about you. They can usually only share information from your records by letter, e-mail, fax or phone. At times, this can be slow and sometimes things get lost on the way.

If a patient has an SCR, they are consulted on a case-by-case basis about whether to allow authorised healthcare staff to view the details. This is referred to as giving permission to view (PTV). Clinicians will only access the record without permission if the patient is unable to give their consent in an emergency (for example, if the patient is unconscious).

The information currently contained within your Summary Care Record is:- Medication Any Allergies Any Adverse Reactions.

You can, as a patient, choose not to have a Summary Care Record. This means that a hospital doctor, for example, would not be able to see your records, even if you wanted them to.

If you would prefer to opt-out, you will need to sign a consent form, please click here or ask at reception for a form.
